Thoracoscopy and Massive Hemothorax in Hemodynamically Stable Patients
نویسنده
چکیده
Trauma is the leading cause of death and disability in the labor force in our country, causing economic losses and prolonged disability. The cardiothoracic trauma is responsible for about 75% of deaths attributed to trauma, penetrating injuries by bladed weapon and firearm projectile generate mortality ranging between 3 and 20% depending on the mechanism of trauma being the most lethal last. In Colombia, the mortality rate is from 3 to 10% inchest wounds stab and from 14 to 20% by firearms, with an incidence of thoracic trauma by blunt trauma 4% and 96% penetrating injuries [1-8].

Background: Penetrating thoracoabdominal stab wounds may cause diaphragmatic and abdominal organ laceration. However, 15-20% of these cases who are stable and managed by conservative treatment might have hidden diaphragmatic injuries, which could ultimately lead to chronic diaphragmatic hernia. Therefore, a safe and exact diagnostic method for the detection of occult diaphragmatic injuries is v...

The aim of this study was to evaluate the diagnostic role and therapeutic effectiveness of videothoracoscopy in chest trauma. Between 1st January 1993 and 30th September 2003, 112 traumatized patients underwent a videothoracoscopy. The origin of trauma was different: 60 road accidents, 42 accidental falls, 7 knife wounds and 3 firearm wounds. Seventy-six patients presented hemothorax, 21 hemopn...
متن کاملMassive hemothorax in a beta-thalassemic patient due to spontaneous rupture of extramedullary hematopoietic masses: diagnosis and successful treatment.
Intrathoracic extramedullary hematopoiesis is an unusual but well-described entity. The condition is typically found in patients who have chronic hemolytic anemias, especially thalassemia. We report a case of a 38-year-old man with underlying beta thalassemia/hemoglobin E who developed intrathoracic extramedullary hematopoiesis. The hematopoietic masses spontaneously ruptured, resulting in mass...

A 39-year-old man had the history of b-thalassemia intermedia and had undergone splenectomy in his childhood. Intrathoracic extramedullary hematopoiesis (EMH) mass was noted on routine follow-up image (Figure 1a). One day, he suffered from acute onset of right upper back pain with dyspnea. He was then sent to our emergency department, and the chest plain film revealed massive right pleural effu...
متن کاملMassive hemothorax caused by Gelpi retractor during posterior correction surgery for adolescent idiopathic scoliosis: a case report
BACKGROUND CONTEXT Gelpi retractors are used in surgery because they can reduce paravertebral muscle damage during retraction. No pleural injuries associated with their use in posterior spine surgery have been reported. PURPOSE To describe a patient who suffered a massive postoperative hemothorax caused by a Gelpi retractor used during posterior correction surgery for adolescent idiopathic sc...
